How do I make an appointment?

What will I need for the first appointment?
Please deliver the intake forms (see Forms tab) to the office at least five days prior to your appointment.  If any lab tests have been done during the year prior to your appointment, please include those results. This will give us time to better structure your initial visit here.

What is your payment policy?
Due to the unconventional  financial approach at this office and the excessive constraints placed on a small office by insurance companies, payment needs to be received at the time (same day) as your office visit.  To keep overhead low we are not able to bill you.  You can pay by cash, check, or money order. Unfortunately, a $25.00 fee for checks returned due to “insufficient funds” will need to be collected from you.  Don’t worry, you will not be judged!  It can happen to anyone!

How do you handle Insurance? 
We have chosen to no longer have a relationship with all insurance companies, including Medicare and MaineCare.  This again has to do with keeping overhead low, lack of fair reimbursement for Functional Medicine services, and other regulatory practices by insurance companies that we believe compromise quality of care.

Can I get reimbursement from my insurance company?
This will depend on your contract with the insurance company. In most cases you should expect fair reimbursement for the services provided to you at this office.  In some cases insurance companies do not consider some of the functional medicine services reimbursable.  Medicare will not reimburse you for care delivered by a non-participating medical provider.  We have opted out of Medicare so this would apply to our office.  When your insurance does reimburse we are not able to guarantee how much you would receive.

How do I handle Insurance Company disputes?
Your contract is between you and the insurance company.  We are not able to get involved in disputes with your insurance company.

We will provide you with a charge slip that includes appropriate diagnostic and therapeutic codes that you can submit to your insurance. We are not able to provide requests for medical records to your insurance company.

Missed Appointments
If you are unable to keep your schedule appointment, we need to have a 24 hour notification from you. There will be a charge for missed appointments without proper notification.
